Kelly’s Kids Home Daycare works with a licensing agency, London Children’s Connection, as well as offering care to private children.
As such there are two registration processes depending on which care option your family decides on.
All families with children in care must read and sign Kelly’s Kids Policies and Procedures and, if interested, sign to consent for the use of the HiMama App.
Private Care
The following forms must be filled out before your child can start private care at Kelly’s Kids:
- Registration Form
- Child Information Form
- Health History Form
- Medication Authorization and Administration Form (if required)
- Infant Feeding Instructions Form (for children less than 12 months of age)
- Special Diet Feeding Instructions Form (for children with special dietary needs)
- Consent Form for sunscreen, diaper creams, sleep arrangements, outdoor supervision, and water use.
(*Please note that all children, regardless of age, shall be directly supervised at all times while outdoors.)

Subsidy
